Care facility owner at center of death investigation owns other low rating facilities

The company that owns a Metro East nursing home where a woman was found dead owns several other nursing homes that have received poor ratings from authorities.

Senior Health Care Management owns Midwest Rehabilitation and Respiratory Center in Belleville, where Juanita Simmons was found dead in a wheelchair at the bottom of stairs. Her death is under investigation. The company also owns 12 other nursing homes in Illinois, most of them are low ranking compared to other facilities.

Medicare gave just one out of five stars to Midwest Rehabilitation and Respiratory Center, both the staffing and inspections rank well below the norm. Inspectors have found 13 problems in past two state inspections, they have also received 11 complaints. In 2014, a nurse pleaded guilty for failing to feed and give patients medication.

In total, nine nursing homes owned by Senior Health Care Management received a one star rating. News 4 attempted to reach out to the company, but nobody returned repeated phone calls.
