(KMOV.com) - A government memo is calling out General Motors about the recent recall that affected 2.6 million vehicles because an ignition switch defect.

The memo states that GM discussed two separate fixes for the ignition defect, but canceled both without taking action.

Engineers met in 2005 to consider making changes to the switch, but the engineer said the switch was fragile and advised against making changes, according to the memo. Later in 2005, GM also approved, but ultimately canceled, a change to the key design.

The ignition defect is linked to 13 deaths.
