I talked to Clint Arthur today. A guy in his mid-40s who lives in Las Vegas and wants to change the way we live our lives for the better.

Besides setting goals we want to achieve in 2010 and actually achieving them (say running a marathon, writing a book, recording a song, going on that dream vacation, attending a championship sporting event, etc) the idea behind living like it's the last year of your life is to resolve personal issues and make peace with who you are.
I asked Clint if he thought we should live like this every year. He told me it takes a lot of energy and commitment to do this and it's something you really have to take one year at a time. He says it's a one-year program for a reason.
